Type 2 diabetes (most common in cats)

Approximately 80 to 95 percent of diabetic cats have Type 2 diabetes. In this form, the pancreas still produces insulin, but the body's cells have become resistant to its effects. The cells do not respond properly to insulin's signal to absorb glucose, so the pancreas has to produce more and more insulin to get the same effect. Eventually, the overworked beta cells in the pancreas become exhausted and damaged, and insulin production falls below the level needed to keep blood sugar under control.

Other breeds with reported increased risk include Siamese, Norwegian Forest Cats, Russian Blues, and Abyssinians. Domestic Shorthairs and Domestic Longhairs, being the most common cats, account for the largest total number of cases simply by population size.

Medications

Certain medications can trigger or worsen diabetes in predisposed cats. Even though your cat is eating well or even eating more than usual, they are losing weight because the body cannot use the glucose from food. Instead, the body starts breaking down fat and muscle for energy, leading to visible weight loss, muscle wasting, and a bony feel along the spine and hips

Other signs to watch for

Beyond the classic four signs, diabetic cats may show additional symptoms:

  • Decreased activity and lethargy: Your cat may sleep more, play less, and seem generally less engaged. Energy metabolism is disrupted, leaving the cat fatigued
  • Poor coat quality: The coat may become dull, dry, unkempt, or develop a greasy or flaky appearance. Some diabetic cats stop grooming as effectively, adding to the coat deterioration
  • Plantigrade stance (walking flat-footed): This is a distinctive sign of diabetic neuropathy, a complication that occurs when prolonged high blood sugar damages the nerves in the hind legs. When diabetes becomes an emergency: diabetic ketoacidosis

Diabetic ketoacidosis (DKA) is a life-threatening emergency that occurs when diabetes goes untreated or becomes severely uncontrolled. When the body cannot use glucose, it turns to breaking down fat at an accelerated rate. This produces byproducts called ketones, which are acidic. Several commercial wet foods are very low in carbohydrates and can work well for diabetic cats. The transition to a new diet should be gradual to avoid digestive upset, typically over 7 to 10 days.

Weight management

For overweight diabetic cats, achieving a healthy weight is one of the most impactful things you can do. Weight loss directly improves insulin sensitivity, meaning the same dose of insulin becomes more effective as your cat loses weight. Some cats achieve remission through weight loss and dietary changes alone or require progressively lower insulin doses as they slim down.

However, weight loss in a diabetic cat must be gradual and carefully managed. Rapid weight loss can trigger hepatic lipidosis (fatty liver disease), a potentially fatal condition in cats. Aim for a loss of no more than 1 to 2 percent of body weight per week, and work closely with your vet to adjust insulin doses as your cat's weight and insulin needs change.

Clinical signs: The most basic and important monitoring is observing your cat's behavior. When diabetes is well controlled, the excessive thirst, urination, and appetite should resolve, weight should stabilize or increase, energy levels should improve, and your cat should seem like their old self again. If these signs return or worsen, it suggests blood sugar is not adequately controlled and an adjustment may be needed.

Home blood glucose testing: Many owners learn to test their cat's blood glucose at home using a small glucometer and a tiny prick of the ear tip. Home testing provides much more accurate readings than in-clinic testing because the cat is relaxed and not affected by stress hyperglycemia. Some owners perform periodic blood glucose curves at home (testing every 2 hours over 12 to 24 hours) to give their veterinarian detailed information about how the insulin is working throughout the day. Blood glucose curves: This involves measuring blood glucose every 2 hours over a 12-hour period (typically during the day between insulin injections) to see how blood sugar rises and falls in response to insulin. This information helps your vet determine whether the insulin type, dose, and timing are appropriate. Curves may be performed in the clinic, though at-home curves are often preferred because stress does not distort the results.

Fructosamine: Rechecked every 2 to 4 weeks initially, then every 3 to 6 months once stable. Fructosamine provides a two-to-three-week average of blood glucose and is helpful for tracking overall trends and response to treatment.

Routine bloodwork and urinalysis: Regular comprehensive bloodwork (every 3 to 6 months) monitors kidney function, liver enzymes, electrolytes, and checks for concurrent conditions. Urinalysis monitors for glucose, ketones, and urinary tract infections.

Understanding hypoglycemia: the most dangerous complication

Hypoglycemia (dangerously low blood sugar) is the most serious acute risk of insulin therapy and can be life-threatening. It occurs when too much insulin is given relative to the amount of glucose available, whether from an insulin overdose, a missed or vomited meal, unusually high activity, or the cat's diabetes going into remission (so they no longer need as much insulin).

Signs of hypoglycemia include:

  • Weakness or lethargy
  • Wobbling, staggering, or unsteady gait
  • Trembling or muscle twitching
  • Disorientation or confusion
  • Hunger or unusual vocalization
  • Seizures (in severe cases)
  • Loss of consciousness (in severe cases)

If you suspect hypoglycemia, act immediately: rub corn syrup, honey, or sugar water on your cat's gums (even if they are unconscious, as it absorbs through the gum tissue). Offer food as soon as your cat is alert enough to eat. Do not give insulin. Contact your veterinarian or emergency clinic immediately. Living with a diabetic cat: practical tips

Managing a diabetic cat becomes routine faster than most owners expect. Here are practical tips from veterinarians and experienced owners:

Establishing a routine

Consistency is the foundation of good diabetes management. Feed and give insulin at the same times each day. Most cats do well with a twice-daily schedule: feed breakfast, give insulin, then repeat approximately 12 hours later for dinner. Set alarms on your phone if needed. If you need to adjust the timing on a given day, a variation of an hour or two is generally acceptable, but try to keep the schedule as regular as possible.

Managing multiple cats
