Hyperthyroidism in senior cats: symptoms, treatment options and care
Hyperthyroidism is the most common hormone disorder in older cats. Learn to recognize the signs and understand treatment options from medication to radioiodine.
If your older cat is suddenly ravenous yet losing weight, racing around the house like a kitten, or drinking water like they just crossed a desert, hyperthyroidism may be the culprit. This hormonal disorder is the most common endocrine disease in cats, affecting approximately 10% of cats over the age of 10.
The good news is that hyperthyroidism is highly treatable. With proper management, most hyperthyroid cats return to their normal selves and can enjoy excellent quality of life for years.
What is hyperthyroidism?
Hyperthyroidism occurs when the thyroid glands, located in the neck, produce excessive amounts of thyroid hormone. These hormones regulate metabolism, essentially controlling how fast the body burns energy. When thyroid hormone levels are too high, the body's metabolism goes into overdrive.
In almost all cases (about 97-99%), hyperthyroidism in cats is caused by a benign (non-cancerous) growth called an adenoma on one or both thyroid glands. This growth autonomously produces thyroid hormone without regard to the body's actual needs. Thyroid cancer causes hyperthyroidism in only 1-3% of cases.
The exact reason why cats develop these growths remains unclear, though research has investigated potential links to canned food, flame retardants in the environment, and certain chemicals in cat litter. No definitive cause has been established.
How common is it?
Hyperthyroidism was rarely diagnosed before 1980, but it has become increasingly common since then. Today, it affects roughly 10% of cats over age 10 and becomes even more prevalent with advancing age. The reason for this increase is debated: it may reflect better diagnostic detection, longer-living cats, or genuine environmental factors.
The condition affects all breeds, though Siamese and Himalayan cats appear to have somewhat lower rates. Male and female cats are equally affected. If you have a cat over 10 years old, hyperthyroidism screening should be part of their regular wellness care.
Recognizing the symptoms
The symptoms of hyperthyroidism result from the body running in "high gear" constantly. While individual cats vary, common signs include:
Weight loss despite increased appetite
This is the hallmark symptom. Your cat may be constantly hungry, begging for food, eating voraciously, even stealing food, yet still losing weight. The accelerated metabolism burns calories faster than your cat can consume them, leading to muscle wasting and fat loss.
Hyperactivity and restlessness
Many hyperthyroid cats seem to have endless energy. They may pace, vocalize excessively (especially at night), appear agitated, or seem unable to settle down. Some owners initially mistake this for their senior cat "acting young again," but it's actually a symptom of disease.
Increased thirst and urination
You may notice your cat drinking more frequently or the litter box containing larger urine clumps. Increased thirst can also indicate kidney disease or diabetes, making veterinary evaluation essential.
Vomiting and diarrhea
The rapid metabolism affects the gastrointestinal tract, leading to frequent vomiting and sometimes diarrhea or increased stool volume. Food may pass through too quickly to be properly absorbed.
Poor coat condition
The fur may become unkempt, matted, or greasy because hyperthyroid cats often groom less. Some cats over-groom certain areas instead, leading to patchy fur loss.
Rapid heart rate and heart murmur
Excess thyroid hormone speeds up the heart and can cause a detectable heart murmur. Prolonged hyperthyroidism can lead to a condition called thyrotoxic cardiomyopathy, where the heart muscle thickens and weakens.
Less common symptoms
Some cats experience weakness, difficulty breathing, or seek out cool places due to heat intolerance. A small percentage of cats show the opposite of typical signs: they become lethargic, lose their appetite, and seem depressed. This presentation, called apathetic hyperthyroidism, can be harder to diagnose.
Diagnosis
Hyperthyroidism is diagnosed through a blood test measuring Total T4 (thyroxine), the primary thyroid hormone. In most hyperthyroid cats, the T4 level is elevated above the normal range. This test is inexpensive, widely available, and part of routine senior bloodwork panels.
However, some cats with hyperthyroidism have T4 levels in the high-normal range, especially in early stages of the disease or if they have concurrent illness. If your cat has classic symptoms but a normal T4, your veterinarian may recommend:
- Repeating the test in 2-4 weeks (T4 levels fluctuate)
- A Free T4 test, which is more sensitive
- Testing T3 levels
- A TSH suppression test or thyroid scan in difficult cases
Your vet will also assess your cat's overall health through physical examination, complete blood count, chemistry panel, and urinalysis. These tests help identify complications of hyperthyroidism and other conditions that may affect treatment decisions.
Treatment options
Four treatment options exist for hyperthyroidism, each with distinct advantages and disadvantages. The best choice depends on your cat's overall health, your preferences, and practical considerations like availability and cost.
Daily medication (methimazole)
Methimazole (brand names Tapazole, Felimazole) is the most common initial treatment. This medication blocks the thyroid gland from producing excess hormone but does not treat the underlying tumor. It must be given for life.
Pros: Non-invasive, relatively inexpensive initially, reversible if problems occur, and allows monitoring of kidney function before committing to permanent treatment.
Cons: Requires daily medication (pill, liquid, or transdermal gel applied to the ear), needs regular blood test monitoring, can cause side effects (vomiting, appetite loss, lethargy) in some cats, and costs accumulate over time.
About 15-20% of cats experience side effects from methimazole, most of which resolve with dose adjustment or switching to transdermal application. More serious side effects affecting the liver or bone marrow are rare but require stopping the medication.
Prescription diet (Hill's y/d)
Hill's Prescription Diet y/d is an iodine-restricted food. Since the thyroid needs iodine to produce hormone, limiting dietary iodine reduces hormone production. The cat must eat only this food, with no treats, hunting, or other food sources.
Pros: No medication required, non-invasive, eliminates the stress of pilling.
Cons: Only works if the cat eats nothing else (challenging in multi-cat households or with outdoor cats), some cats refuse the food, does not treat the underlying tumor, and long-term iodine restriction may have unknown effects.
The dietary approach works well for some cats and their owners but requires strict compliance that is not realistic for every household.
Radioactive iodine (I-131)
Radioactive iodine treatment is considered the gold standard and is the only true cure for hyperthyroidism in most cases. A single injection of radioactive iodine is taken up by the overactive thyroid tissue and destroys it, sparing healthy thyroid tissue.
Pros: Curative in 95-98% of cases with one treatment, no daily medication needed afterward, treats both thyroid glands simultaneously, minimal side effects.
Cons: Higher upfront cost ($1,000-$2,500 depending on location), requires specialized facility, cat must be hospitalized for 3-14 days until radiation levels are safe, not appropriate for cats with severe kidney disease.
Most cats tolerate the treatment extremely well and go on to live normal lives. A small percentage become hypothyroid (underactive thyroid) afterward and require thyroid supplementation.
Surgery (thyroidectomy)
Surgical removal of the affected thyroid gland(s) was once the standard treatment but has largely been replaced by radioactive iodine because of lower risk and higher success rates with I-131. Surgery may still be appropriate when radioactive iodine is unavailable or contraindicated.
Pros: Can be curative, tumor is removed and can be biopsied.
Cons: Requires anesthesia in often older, compromised patients, risk of damage to parathyroid glands (which control calcium), may not remove all abnormal tissue, higher complication rate than I-131.
The critical connection with kidney disease
Hyperthyroidism and chronic kidney disease commonly coexist in senior cats, and their relationship is important to understand. Here's why: hyperthyroidism increases blood flow to the kidneys, which can mask underlying kidney disease by making kidney values appear better than they actually are.
When hyperthyroidism is treated and thyroid levels normalize, kidney function may appear to decline, not because treatment caused kidney damage, but because treating the hyperthyroidism reveals kidney disease that was already present but hidden.
For this reason, many veterinarians start with a trial of methimazole medication before committing to permanent treatments like radioactive iodine. This allows monitoring of kidney function after thyroid levels are controlled. If kidney values remain stable, permanent treatment can proceed. If significant kidney disease emerges, the cat may be better served by keeping hyperthyroidism partially controlled to protect kidney function.
Your veterinarian will help navigate this balance, weighing the risks of untreated hyperthyroidism (heart disease, weight loss, poor quality of life) against the risks of fully treating it in a cat with compromised kidneys.
Monitoring and follow-up care
Regardless of which treatment you choose, ongoing monitoring is essential:
- For cats on medication: Blood tests every 3-6 months to check thyroid levels and kidney function, with dose adjustments as needed
- For cats on prescription diet: Regular blood tests to confirm the diet is controlling thyroid levels
- After radioactive iodine: A recheck at 1-3 months, then typically annual monitoring
- After surgery: Post-operative monitoring of calcium and thyroid levels, then ongoing monitoring
Watch for recurrence of hyperthyroid symptoms (weight loss, increased appetite, hyperactivity) which could indicate treatment failure or, in the case of radioactive iodine, regrowth of abnormal tissue.
Secondary effects and complications
Uncontrolled hyperthyroidism can cause several serious complications:
- Heart disease: The heart enlarges and can fail if hyperthyroidism is not treated; fortunately, thyroid-related heart disease often improves or resolves with treatment
- High blood pressure: Can damage the eyes, kidneys, heart, and brain; typically improves with hyperthyroid treatment but may require separate medication
- Blindness: From retinal detachment due to high blood pressure
- Severe weight loss: And muscle wasting that weakens the cat significantly
These complications underscore the importance of treating hyperthyroidism rather than assuming an older cat is "just slowing down."
Prognosis
The prognosis for hyperthyroid cats is generally excellent with appropriate treatment. Cats treated with radioactive iodine are often cured permanently. Cats managed with medication or diet can do well for years with proper monitoring.
Life expectancy depends largely on the cat's age at diagnosis, the presence of other diseases (particularly kidney disease), and how well the hyperthyroidism responds to treatment. Many cats live 3-5 additional years or more after diagnosis, with good quality of life.
The small percentage of cats (1-3%) with thyroid cancer have a less favorable prognosis, as cancer can spread and may not respond to standard treatments.
Living with a hyperthyroid cat
Once your cat's hyperthyroidism is controlled, you can expect a return to normal behavior: stable weight, normal appetite, calmer demeanor, and improved coat condition. Regular veterinary visits and blood tests become part of your routine.
If giving daily medication, work with your vet to find the easiest administration method for your cat, whether that's pills, flavored liquid, or transdermal gel. Many cats adapt well to a routine.
Keep an eye out for any return of symptoms, which could indicate the need for dose adjustment or additional evaluation.
Hyperthyroidism is one of the most treatable conditions affecting senior cats. If you notice symptoms in your older cat, don't wait to see your veterinarian. Early diagnosis and treatment lead to the best outcomes and can restore your cat's quality of life.
