How pancreatitis is diagnosed

Physical examination
Your vet will perform a thorough physical exam, paying close attention to hydration status, body condition, abdominal palpation (feeling the belly), and checking for fever, jaundice, and signs of pain. However, the physical exam alone is rarely diagnostic for pancreatitis in cats because the signs are so nonspecific.
Blood tests
Feline Pancreatic Lipase Immunoreactivity (fPLI or Spec fPL): This is the most useful blood test for diagnosing pancreatitis in cats. It measures the level of a lipase enzyme that is specific to the pancreas. An elevated Spec fPL is highly suggestive of pancreatitis and has become the cornerstone of diagnosis. The test is widely available and can be run as a quick in-house SNAP test (which gives a normal or abnormal result within minutes) or sent to a reference laboratory for a precise numerical value.
It is worth noting that the Spec fPL test is not perfect. It can be falsely negative in some cases of pancreatitis (particularly mild chronic cases) and can sometimes be elevated in cats without pancreatitis. It is most useful when interpreted alongside the cat's clinical signs, other lab results, and imaging findings.
Complete blood count (CBC): May show changes consistent with inflammation or infection, though results are often normal in mild cases. In severe pancreatitis, you may see elevated white blood cell counts or changes suggesting a systemic inflammatory response.
Chemistry panel: Evaluates liver enzymes (often elevated due to concurrent liver involvement or bile duct obstruction), kidney values (important for managing fluid therapy), blood glucose (diabetes can be concurrent or can develop as a result of pancreatitis), and electrolytes.
Imaging
Abdominal ultrasound: This is the most useful imaging tool for pancreatitis and is often performed alongside blood tests. In pancreatitis, the ultrasound may show an enlarged or hypoechoic (darker than normal) pancreas, fluid surrounding the pancreas (peripancreatic effusion), abnormalities of the bile duct or liver, and signs of intestinal inflammation. However, a normal ultrasound does not rule out pancreatitis, especially in chronic or mild cases. The sensitivity of ultrasound for feline pancreatitis ranges from 11 to 67 percent depending on the study and the skill of the ultrasonographer. When changes are present, though, ultrasound is highly valuable for assessing severity and checking for complications.
Abdominal radiographs (X-rays): Generally not very helpful for diagnosing pancreatitis itself, but they can rule out other causes of the cat's symptoms such as intestinal obstruction, masses, or urinary stones.
Biopsy
Definitive diagnosis of pancreatitis requires histopathology (microscopic examination of pancreatic tissue), but biopsies are rarely performed in practice because they require surgery or laparoscopy, which carries significant risk for an already sick cat. Biopsies are typically reserved for cases where cancer needs to be ruled out or where the diagnosis remains uncertain despite other testing.
The diagnostic challenge
Because no single test is 100 percent sensitive or specific, diagnosing pancreatitis in cats often comes down to a combination of compatible clinical signs, elevated fPLI, supportive ultrasound findings, and ruling out other causes of the symptoms. Your veterinarian may diagnose "suspected pancreatitis" rather than confirmed pancreatitis, and treatment is often started based on strong clinical suspicion even when test results are not perfectly clear-cut.
Treatment: supporting recovery
There is no specific cure for pancreatitis. Treatment focuses on controlling pain, managing nausea, maintaining nutrition and hydration, addressing concurrent conditions, and giving the pancreas time to heal. The intensity of treatment depends on the severity of the episode.
Pain management
Adequate pain control is arguably the single most important aspect of pancreatitis treatment. Pancreatitis is painful, even when cats do not show obvious signs of pain. Uncontrolled pain worsens nausea, suppresses appetite, slows recovery, and causes suffering. Yet historically, pain has been undertreated in cats with pancreatitis because cats hide their discomfort so effectively.
Common pain management options include:
- Buprenorphine: An opioid pain reliever that can be given by injection in the clinic or as a dissolvable tablet placed on the gums at home. It provides moderate to good pain relief and is generally well tolerated
- Gabapentin: Increasingly used for pain management in cats, often in combination with other pain medications. It also has mild sedative and anti-anxiety effects, which can be beneficial for stressed, painful cats
- Methadone or fentanyl: Stronger opioids that may be used in hospitalized cats with severe pain
- Solensia (frunevetmab): A monthly injectable anti-nerve growth factor antibody approved for pain in cats. While primarily used for osteoarthritis pain, some veterinarians use it as part of a multimodal pain management plan
NSAIDs (non-steroidal anti-inflammatory drugs) are generally avoided in cats with pancreatitis, especially those who are dehydrated or have concurrent kidney disease, due to the risk of kidney damage and gastrointestinal side effects.
Anti-nausea medication
Nausea is a major contributor to appetite loss in cats with pancreatitis, and it may be present even when the cat is not actively vomiting. Anti-nausea medications can make a significant difference in the cat's comfort and willingness to eat:
- Maropitant (Cerenia): The most commonly used anti-nausea medication in veterinary medicine. It is highly effective, can be given by injection or orally, and also has some pain-relieving properties in the abdomen

Fluid therapy

- Intravenous (IV) fluids: Used for cats who are significantly dehydrated or severely ill. IV fluids require hospitalization but provide the most effective and rapid rehydration
- Subcutaneous fluids: Fluids given under the skin, which absorb slowly over several hours. This can be done at the clinic or taught to owners for home administration. Subcutaneous fluids are appropriate for mildly to moderately dehydrated cats and are often continued at home during recovery
Nutritional support
Getting your cat eating again is critical. This is one of the most important differences between treating pancreatitis in cats versus dogs. In dogs, the traditional approach was to withhold food to "rest the pancreas." In cats, prolonged fasting is dangerous because it can trigger hepatic lipidosis (fatty liver disease), a potentially fatal condition in which the liver becomes overwhelmed by fat mobilized from the body's stores during starvation.
Current veterinary guidelines emphasize early nutritional support:
- Appetite stimulants: Mirtazapine (oral or transdermal) and capromorelin (Elura) can help stimulate appetite in cats who are feeling well enough to eat but lack interest
- Syringe feeding: If your cat will not eat voluntarily, your vet may recommend carefully syringe-feeding small amounts of a highly digestible, calorie-dense recovery diet. This must be done gently to avoid aspiration and stress
- Feeding tubes: In cats who refuse to eat for more than 48 to 72 hours, or who are at high risk for hepatic lipidosis, your veterinarian may recommend placing a feeding tube. While this sounds dramatic, feeding tubes are extremely well tolerated by cats and can be lifesaving. Common types include esophagostomy (E-tubes) and nasogastric tubes. They allow you to deliver nutrition, water, and medications directly without the stress of force-feeding
- Diet choice: Highly digestible, moderate-fat, high-quality protein diets are generally recommended. Unlike dogs, where low-fat diets are a cornerstone of pancreatitis management, there is no strong evidence that fat restriction is necessary or beneficial for cats with pancreatitis. Cats are obligate carnivores who metabolize fat differently than dogs, and most veterinary nutritionists focus on palatability and caloric density rather than strict fat restriction
Treating concurrent conditions
Because pancreatitis in cats so often occurs alongside other diseases, comprehensive treatment needs to address the whole picture:
- IBD: If inflammatory bowel disease is contributing to or coexisting with pancreatitis, dietary management (novel protein or hydrolyzed diets) and possibly immunosuppressive medications like prednisolone may be needed. The use of corticosteroids in cats with pancreatitis is controversial but is generally considered safer in cats than in dogs, and some cats with concurrent IBD and pancreatitis respond well to judicious steroid use
- Cholangitis (liver inflammation): May require antibiotics (if a bacterial component is suspected), ursodiol to improve bile flow, and supportive care for the liver
- Diabetes: If pancreatitis has caused or is complicating diabetes, insulin therapy needs to be carefully managed, as insulin requirements can fluctuate significantly during pancreatitis flare-ups
- Hepatic lipidosis: If prolonged anorexia has triggered fatty liver, aggressive nutritional support (often via feeding tube) becomes the primary treatment priority
Hospitalization vs. home care
The decision to hospitalize depends on severity:
- Mild cases: Cats who are still eating some food, are not severely dehydrated, and are not in significant pain may be managed as outpatients with oral medications, subcutaneous fluids, and close monitoring at home
- Moderate cases: Cats who have stopped eating, are moderately dehydrated, or are in significant pain typically benefit from 24 to 72 hours of hospitalization for IV fluids, injectable pain medications, and monitoring

Recovery and home care

Chronic pancreatitis: the long game
Many senior cats develop chronic pancreatitis, a condition that waxes and wanes over time rather than resolving completely. Managing chronic pancreatitis is less about curing the disease and more about minimizing flare-ups, maintaining quality of life, and monitoring for progression.
Living with chronic pancreatitis
Cats with chronic pancreatitis may have periods of weeks or months where they seem perfectly fine, punctuated by flare-ups where symptoms return. Over time, owners learn to recognize the early signs of a flare, such as a slight dip in appetite, increased sleepiness, or subtle behavioral changes, and can intervene early with supportive care and veterinary guidance.
Long-term management strategies include:
- Consistent, high-quality diet: Feed a highly digestible diet your cat enjoys. Consistency helps minimize digestive stress on the pancreas. Avoid sudden diet changes

- Vitamin B12 supplementation: Chronic pancreatitis (especially when combined with IBD) can impair absorption of cobalamin (vitamin B12), leading to deficiency. B12 deficiency causes further appetite loss, lethargy, and poor recovery. Your vet may recommend regular B12 injections or high-dose oral supplementation
Potential long-term complications

- Chronic pancreatitis: The long-term prognosis depends on the degree of damage, the presence of concurrent diseases, and how well flare-ups are managed. Many cats with chronic pancreatitis live for years with good quality of life with appropriate ongoing care
The single most significant prognostic factor in many cases is whether the cat will eat. Cats who maintain or quickly regain their appetite have dramatically better outcomes than those who become anorexic. This is why nutritional support is such a central part of treatment.
Prevention: reducing the risk of flare-ups
While the idiopathic nature of most feline pancreatitis makes true prevention difficult, you can take steps to reduce the risk and frequency of episodes:
